The health professional attending the home visit:<\/em><\/strong><\/p> Contacts the patient<\/p> Obtains the patients’ consent to come to their home to prescribe a home exercise program<\/p> Makes an appointment for the home visit \u2013 inform the patient to allow approximately 1.5 \u2013 2.0 hours<\/p> Explains the program<\/p><\/blockquote> Before the decision is made to proceed with home-based rehabilitation, the following needs to be addressed:<\/em><\/strong><\/p> The visiting health professional must have a mobile phone and a car<\/p> Ascertain language skills and cognitive ability of the patient<\/p> Does the patient have a dog or other animals that may be an issue during the assessment<\/p> Review the \u00a0Working safely in visiting health services resource\u00a0(2.4 pages 11-18)<\/span><\/a><\/p><\/blockquote> Security:<\/em><\/strong><\/p> The security of the home assessor must be strictly monitored. As well as completing the \u201cHealth Workers Safety<\/a>\u201d check list prior to the home visit, the following options should be considered:<\/p> Attend with a colleague<\/p> Send a colleague an SMS message which includes the patients\u2019 address, time of arrival and anticipated completion time<\/p> Message again at the completion of the visit once they have left the premises<\/p> Provide the health professional with a duress alarm (eg Work Safe Guardian<\/a>)<\/p><\/blockquote>
