Post Program Assessment

Following the program, the patient will be invited to return to the hospital or community health centre for a post program assessment.

This will include all of the tests that were performed prior to the commencement of the program:

Field walking test

Quality of life questionnaires

Dyspnoea scale e.g Borg Dyspnoea Scale

Measure of anxiety and depression

Self efficacy

The results of these tests will assist the health care team in evaluating the effectiveness of the program.

The pre and post program results should be sent to the referring health professional. See sample Letter to referrer
