Referrals

Referring patients to a program

Referrals to pulmonary rehabilitation can be made to the program coordinator by a health professional or the potential participant (self-referral).

For self-referrals it is advisable for the program co-ordinator to contact the patient’s GP, or respiratory specialist, to obtain a medical history.

referral form for the program will provide the required information.

Following referral to the program the patient is contacted to arrange a centre based assessment (generally held in a hospital or community centre). Contact can be via a phone call or Assessment Invitation.

The invitation letter should include:

An explanation that the patient has been referred to the program

An assessment appointment
